    Belgium's healthcare system is based on mandatory health insurance and a hybrid model of public and private providers.     Belgium's life sciences R&D funding is decentralized across Flanders, Wallonia and the Brussels-Capital Region.     Belgium ranked #2 in the European Economic Area (EEA) for per-capita commercial trial starts in 2023 (1.84 per 100,000), but the European Federation of Pharmaceutical Industries and Associations (EFPIA) notes a decline in immunization-trial starts since 2018 and stakeholder concerns about longer approval timelines post-EU Clinical Trials Regulation (CTR). Belgium ranks in the top 3 in Europe for clinical trials, with around 629 applications submitted between January 2022 to January 2023. About 80% of trial requests originate from biopharma companies; in 2021, Belgium accounted for 20% of Europe's cancer-related trials.

    Life sciences are Belgium's leading export category, with biopharmaceutical exports valued at around $21.65 billion (€18.6 billion). Nuclear medicine also contributes about $262 million (€225 million) in added value, supporting 5,000 jobs nationally.     According to the Global Innovation Index 2024, Belgium had 6,964 researchers per million people in 2022, ranking 7th worldwide. In Wallonia, the sector employed over 20,000 people across more than 260 companies, with 11,000 researchers and an average 4.5% annual employment growth between 2005 and 2022. As of 2022, Belgium's biopharma sector directly employed 43,000 people, and including suppliers and supporting industries, the total impact rises to over 137,000 jobs, roughly three times the European average per capita.
